Thank you for choosing Minnesota Sport and Spine. As part of the registration process, you will need to complete a couple of forms. Filling these forms out in advance will greatly expedite the check-in process at your first visit. All patients must complete the PHYSICAL THERAPY NEW PATIENT PACKET prior to their first appointment. Please note that when initialing/signing the Authorization to Treat form, you are initialing that you have received a copy of our policies. Please review the documents under the “POLICIES” link prior to signing.

A Parent or legal guardian must provide us with written consent in order for us to treat any patient under the age of 18 without a parent or guardian present.

We are providing you with a copy of our office policy as well as the HIPAA Privacy Policy. These do not need to be printed or signed, but should be reviewed prior to signing the Authorization to Treat form included in the registration packet.
